What to Know Before Your Visit

Whether you’re a new patient or coming back for a procedure, we want your experience at Prism Oral Surgery & Implants of Summit to be smooth, informed, and stress-free. Below, you’ll find everything you need to prepare for your appointment — including what to bring, what to expect, and how we handle insurance and payments.

What to Bring

Please arrive 10–15 minutes early and bring the following:

  • A valid photo ID
  • Insurance card(s)
  • Referral slip (if provided by your general dentist or physician)
  • A list of current medications and health conditions
  • Any recent X-rays, if available
  • Your completed new patient forms (if not submitted online)

Insurance and Payment

We will help verify your benefits  and provide an estimate of your out-of-pocket costs. All payment is due at the time of service.  If your insurance is out-of-network, we can still submit the claims for you.

Payment methods accepted:
Cash, credit/debit cards, HSA/FSA, and third-party financing (CareCredit and Cherry).

Sedation &
Comfort Options

We offer several sedation options based on your procedure and preference, including nitrous oxide, oral sedation, IV sedation, and general anesthesia. Sedation will be discussed during your consultation, and if scheduled, you’ll receive clear instructions on how to prepare and what to expect afterward.

New Patient Forms

To save time during your first visit, we encourage you to complete your forms online or download them in advance.

Pre and Post-Op Instructions

Our team is available if you have any questions or concern before, during or after your treatment.  We provide detailed instructions for clarity and understanding.
